Abstract: Estudo epidemiológico descritivo e analítico sobre detecção de casos novos de hanseníase em residentes de São Luís, Maranhão, Brasil, de 1993 a 1998, a partir da elaboração e análise de banco de dados com 2796 notificações. Foram calculados coeficientes de detecção brutos e específicos por sexo, forma clínica e faixa etária. Foi analisada a tendência temporal de tais indicadores através dos modelos de ajuste linear, exponencial, geométrico e logarítmico. Evindenciou-se tendência de crescimento à custas do sexo feminino e das formas paucibacilares, principalmente da tuberculóide. A detecção aumentou na população entre 15 e 59 anos, e mais acentuadamente ainda na faixa de 15 a 19 anos. A detecção em menores de 15 anos teve percentual indicativo de necessidade de busca ativa nesta população e manteve-se hiperendêmica, podendo refletir expansão da doença. Houve baixa freqüência do diagnóstico precoce e predomínio da detecção por demanda espontânea. O registro e exame dermato-neurológico dos comunicantes foi deficiente. Verificou-se inconsistência das informações relativas aos graus de incapacidade e às deformidades apresentados pelos pacientes. A cobertura da terepêutica específica foi adequada. A análise dos indicadores epidemiológicos e operacionais indicou crescimento da detecção hansênica provavelmente mais devido à expansão da endemia que ao fortalecimento de suas atividades de controle. Abstract: We conducted a population-based survey on five small islands in South Sulawesi Province (Indonesia) to collect baseline data previous to a chemoprophylactic intervention study aiming at interrupting the transmission of Mycobacterium leprae. Here we describe the present leprosy epidemiology on these geographically isolated islands. Of the 4774 inhabitants living in the study area 4140 were screened for leprosy (coverage: 87 per cent). We identified 96 leprosy patients (85 new and 11 old patients), representing a new case detection rate (CDR) of 205/10 000 and a prevalence rate of 195/10 000. CDRs were similar for males and females. Male patients were more often classified as multibacillary (MB) than women. Of the new patients, 33 (39 per cent) were classified as MB, 16 (19 per cent) as paucibacillary (PB) 2-5 lesions and 36 (42 per cent) as PB single lesion. In this area of high leprosy endemicity leprosy patients were extensively clustered, i.e. not equally distributed among the islands and within the islands among the houses. (AU). Abstract: A questionnaire was administered to all patients with leprosy seen at the four leprosy clinics in Anambra State in a face to face interview. The questions covered, among other items, the clinic attendance behaviour and the single most important reason, monthly, for absenteeism in the preceding year. The total and individual frequencies of the reasons for absenteeism were determined for the various behavioural subgroups. The differences in frequencies and associations were analysed. Values of P < 0.05 were considered as significant. The results showed that 27 females and 26 males were interviewed. 39.6 per cent of the patients were irregular attenders 735 per cent were defaulters. Attendance at meetings (P < .001); work at home (P < 0.01) fear/shame/indignation (P < 0.05); no confidence in treatment (P < 0.025) were significant reasons for absenteeism among irregular attenders inter-current illnesses as reasons for absenteeism did not differ significantly between regular and irregular attendees. The association between clinic attendance behaviour and lesion location (revealed Vs concealed) was not statistically significant (X(2)0.3). The findings in this study indicate that in the post leprosaria abolition years, default and irregular clinic attendance by patients with leprosy are numerically large and may compound the problems of control programmes, and thus negate the realization of the global goal of intercepting leprosy transmission.
